About the Role
• Conduct comprehensive patient assessments and develop individualized care plans.
• Monitor patient progress, document interventions, and update care plans as needed.
• Educate patients and families on health conditions, treatments, and self-care strategies.
• Coordinate with the interdisciplinary team and communicate with providers regarding patient status.
• Ensure compliance with federal, state, and agency regulations and best practices.
• Provide nursing care including wound care, medication management, and chronic disease education.
• OASIS assessment experience (preferred).
Requirements
Current, unencumbered RN license in the State of Minnesota.
Minimum of [1-2 years] of nursing experience, preferably in home health, acute care, or community health.
Strong clinical assessment and critical thinking skills.
Reliable transportation and valid driver’s license.
Excellent communication, organizational, and documentation abilities.
Ability to work independently in diverse home environments.
